1.) You entered 5 or more wrong codes into your phone. (3 or more on some models)

2.) The phone was bought off Ebay or other auction site and the seller did the same as above.

3.) You tried a code that was generated for someone else's phone and assumed that the code would also work on yours. All IMEI numbers are different, which means unlock codes are like fingerprints, every phone will have its own unique unlock code.

4.) You kept entering the same code over and over when seeing "code error".
